Improving Comfort and Quality of Life
Kelsey-Seybold’s Palliative Care program can improve quality of life in a number of ways. Together with your primary care doctor, our team combines pain and symptom control into every aspect of your treatment. Team members spend time with you and your family so that you all fully understand your condition, care options, and other needs. We also help ensure a smooth transition with external services as needed.
Four Steps to Receiving Care
If you or a loved one might benefit from Palliative Care, talk to your Kelsey-Seybold doctor, who can refer you to our team or request a consultation. Receiving care requires the following steps:
- The patient’s physician will complete a referral to the Palliative Care team. Alternatively, the patient or their loved ones can call our referral line to schedule a consultation yourself.
- The patient or patient’s loved ones will be contacted by the Palliative Care nursing team to schedule an evaluation.
- An initial assessment will be completed to determine which portions of the program the patient would benefit from most.
- The plan of care will be developed and discussed with the patient and loved ones present during the initial evaluation.
